Please make sure that if this is your first visit at Urban Acupuncture Center that you have signed up for a "First Visit" session, this ensures that we have enough time for your consultation and treatment. If you are already established with us, please sign up for the "Return Visit".
Paperwork
**Please go to 'Online Forms' tab from our website, print off, complete, and bring the Initial Intake Paperwork with you to first treatment. If you don't have access to a printer, please come at least 15 minutes early to complete here.**
Cancellation Policy
We ask for 24 hours notice in advance of an appointment if it is necessary to cancel or reschedule that appointment. You can do this by calling us or changing your appointment online.
All appointments that are rescheduled or cancelled with less than 24 hour advance notice and appointments missed without notice will be charged a $15 fee. If appointments have been purchased with a gift certificate or as part of a package, missed, cancelled or appointments rescheduled less than 24 hours ahead of time will be deducted from the number of remaining appointments.
